DXpedition Grant Request Form
Please read the instructions before clicking on the grant form below.
I recommend you fill out the form and save a copy as well as print it before sending.
If you must send additional paperwork and you do not have the availability to scan it then you should print the form, attach all paperwork and send it to KC8CKW.
If you do not have a mail client on your PC, such as Outlook, Outlook Express, Eudora etc., and you use a WEB mail client you will have to save the form and and than attach it in your WEB mail client.
UNIX / LINUX you must download Adobe Acrobat in order to use this form.
This forms has been tested with Acrobat Reader versions 8, 9 & 10. It has also been tested on LINUX UBUNTU version 10.10 with Acrobat version 9.
You must use this form if you want to be considered for a grant, all other submissions will not be processed!
